HIPAA legacy integration patterns for enterprise AI agents — Reference Architecture
The control that resolves this is an Idempotent System-of-Record Bridge, evaluated before the request reaches a model provider rather than audited afterwards. In regulated enterprise the binding constraint is HIPAA, and the question it actually asks is can PHI reach a place the BAA does not cover. The specific problem here is mainframe integration. The shape below holds p95 ≤ 2400ms over scheduled batch file exchange (SFTP), and ships with a standard-library implementation that runs as written.
The request path

- Content-derived idempotency key — Derived from business keys, so two attempts at one intent collide.
- Claim table — Transactional claim in the same database as the outbox; a retry observes the first attempt.
- Typed write contract — Allow-listed fields per document type; unknown fields are rejected loudly.
- Outbox — Durable intent record, so a crash mid-flight is recoverable rather than ambiguous.
- Reconciliation job — Compares posted documents against intents and surfaces drift.
- Compensating action — A defined reversal path per document type, because some writes must be undoable.
Enterprise implementation parameters
Every value below is a parameter you can hold a vendor or an internal team to. Adjectives are not acceptance criteria.
| Deployment vector | On-premises gateway adjacent to the system of record |
|---|---|
| Security standard | HIPAA |
| Applicable regimes | SOC 2 Type II, ISO/IEC 27001:2022, GDPR |
| Latency SLA | p95 ≤ 2400ms — interactive surfaces need sub-second; batch jobs do not |
| Integration protocol | scheduled batch file exchange (SFTP) — the legacy system of record exposes nothing else |
| Idempotency control | a content hash per file plus a processed-files ledger |
| Throughput envelope | a window, not a rate; the constraint is the window length |
| Middleware requirement | policy decision point in front of the model gateway |
| Data classes in scope | PII, commercial-in-confidence data |
| Retention obligation | per the data class retention schedule, enforced automatically |
| Data residency | contracted processing region, asserted on every response |
| Contractual instrument | DPA plus a disclosed subprocessor list |
| Systems of record | SAP, Salesforce, Workday |
| Primary failure mode | a partial file processed as complete |
Where the rollout actually stalls
Agents can read the legacy estate and cannot safely write to it. A retry after a gateway timeout creates a duplicate posting, and in an ERP a duplicate journal entry is a finance incident. So the team that owns the system of record withholds write access, and the automation stalls at read-only.
What makes mainframe integration specifically hard
The mainframe is the system of record and its change window is measured in quarters. Anything that requires a change on that side is not an integration plan, it is a request to join a release queue.
What resolves it is a side-car integration that requires zero mainframe change. The failure to design against: a design whose first step is a CICS change nobody will schedule.
What HIPAA specifically requires
HIPAA holds the covered entity responsible for PHI wherever it flows. A model provider without a signed BAA is not a permissible destination, and 'the prompt only contained a little PHI' is not a defence — the minimum necessary standard is itself a control.
The artifact that satisfies it is a de-identification record per request plus the BAA chain. The way teams most often fail it: PHI reaching a subprocessor with no BAA, via a logging sidecar nobody mapped.
Implementation artifact — Idempotent write-back with a typed write contract
Standard-library Python, no external dependencies, runs as written. Executed by the publish gate: if it raises, this page does not ship. Wire it behind scheduled batch file exchange (SFTP); the failure mode it guards against is a partial file processed as complete. The output you hand the assessor is the minimum-necessary de-identification log.
"""Idempotent write-back adapter for a legacy system of record (SAP/ERP shape).
Why this shape: the blocker on legacy integration is almost never reading. It
is writing. An agent that retries a posting because a gateway timed out will
happily create a duplicate journal entry, and in an ERP a duplicate posting is
a finance incident, not a bug report.
Three controls make agent-driven writes acceptable to the team that owns the
system of record:
* a deterministic idempotency key derived from business content
* a claim table, so a retry observes the first attempt instead of repeating it
* a bounded, typed projection, so the agent can never post a field it does
not understand

The HIPAA control, as code
Minimum-necessary de-identification with a BAA-pinned egress gate. This is the artifact an assessor asks for under HIPAA specifically — it is not the same control as the one above, and a system that implements one does not thereby implement the other. It executes under the same publish gate.
"""Minimum-necessary de-identification with a BAA-pinned destination gate.
HIPAA holds the covered entity responsible for PHI wherever it flows, so the
control is not "we told people not to paste PHI". Two things must be
structural: PHI is reduced to the minimum necessary BEFORE egress, and the
destination set is pinned to those under a signed BAA, failing closed on
anything else.
Safe Harbor (45 CFR 164.514(b)(2)) lists 18 identifier classes. This implements
the mechanical subset and records a per-request de-identification receipt,
because the auditor asks for proof per request, not a policy document.

Risk mitigation matrix
Each row carries an acceptance parameter rather than an adjective, because "low latency" cannot be signed off. Retention here is bound by per the data class retention schedule, enforced automatically.
| Category | Risk | Control | Acceptance parameter |
|---|---|---|---|
| HIPAA | PHI egresses to a provider or subprocessor outside the BAA chain | Classify and de-identify before egress; pin destinations to the BAA-covered set and fail closed on anything else | 0 requests to non-BAA destinations; de-identification applied pre-egress on 100% of PHI-classified payloads |
| mainframe integration | Integration requires a change inside the mainframe release window | Confine all new logic to the side-car; treat the mainframe interface as immutable | 0 mainframe-side changes required to ship; verified in design review |
| Operational | Retry after timeout creates a duplicate document | Business-content idempotency key plus a transactional claim; retries replay the original result | Induced timeout + retry produces 1 document and 1 transport call |
| Operational | Agent posts a field nobody contracted for | Allow-list projection per document type; reject unknown and require complete fields | 0 writes containing fields outside the contract; enforced in tests |
| Operational | Crash mid-flight leaves an ambiguous state | Durable outbox written before transport; reconciliation resolves on restart | Kill-during-post leaves 0 ambiguous records after reconciliation |
| Data privacy | Legacy extracts pull more fields than the use case needs | Projection on read as well as write; field-level justification recorded | Extract schema matches the documented minimum field set |
| Latency SLA | Synchronous legacy calls stall the agent loop | Async submit with a claim handle; agent polls or is called back | Agent p95 independent of the system-of-record response time |
| Cost control | Re-extracting the full estate on every run | Change-data capture or a watermark; full extract only on schema change | Incremental run under 10% of full-extract cost |
| Operational | No defined reversal for a wrong posting | A compensating action per document type, rehearsed in a sandbox client | Every writable document type has a tested reversal path |
Control questions before sign-off
- Which destinations hold a BAA, and what structurally prevents PHI from reaching one that does not?
Destinations carry a BAA flag as attested data, and the routing gate refuses a PHI-classified payload to any destination without it. The refusal is logged with its reason. - What does this require the mainframe team to change, and when is their next window?
Nothing. All new behaviour lives in the side-car and the existing interface is consumed as-is, so delivery does not depend on their release queue. - If the gateway times out and the agent retries, how many documents exist — and what proves it?
One. The idempotency key is derived from the business keys, so both attempts collide on the same claim; the retry observes the completed claim and replays the original result. The transport is invoked once, which the test asserts. - Which exact fields may an agent write per document type, and what happens to a field outside that list?
Only the fields in that document type's allow-list, and all of them — the projection rejects both unknown fields and incomplete payloads. A field outside the contract raises rather than posting hopefully. - What does the system-of-record owner see when a posting is wrong, and what reverses it?
The reconciliation job compares posted documents against recorded intents and surfaces drift; each writable document type has a defined, rehearsed compensating action for reversal. - Where is the intent recorded before the transport call, and what resolves a mid-flight crash?
In the outbox, written before the transport call. On restart, reconciliation resolves anything left in flight, so a crash leaves a recoverable state rather than an ambiguous one. - Is the extract pulling the minimum field set, with a justification per field?
It should be — projection applies on read as well as write, with a recorded justification per field, so the extract schema matches the documented minimum field set.
